Troopers investigate fatal three-vehicle crash in the town of Antwerp

ANTWERP, NY —  State Police is investigating a three-vehicle crash that occurred on State Route 11 in the town of Antwerp.

The investigation has revealed, at approximately 8:26 a.m., a (Fed Ex delivery truck), operated by Stephen T. Kautz,43, Evans Mills, NY, was traveling northbound on State Route 11 in the town of Antwerp when he failed to observe a 2020 Jeep, operated by 58-year-old Tina Marie Stevens stopped in the northbound lane waiting to make a left turn into a driveway and struck the rear of the Jeep causing the vehicle to be pushed into the southbound lane where it was struck by 2020 GMC SUV operated by Sheree L. Cook, 52, Philadelphia, NY.  The Jeep then overturns coming to rest on the west shoulder of the roadway, troopers said.

Stephen T. Kautz was transported to Samaritan Medical Center with non-life-threatening injuries.

Tina M. Stevens was transported by ambulance and went into cardiac arrest as they arrived at Samaritan Medical Center.  She was subsequently pronounced deceased after life-saving measures were unsuccessful.  An autopsy will be performed by the Jefferson County Medical Examiners office to determine the official cause of death.

Sheree L. Cook was transported to Samaritan Medical Center for non-life-threatening injuries.

The investigation is continuing.
